Who kills Captain Rex?

Captain Rex is killed at the end of the 2016 Star Wars Rebels episode “Twilight of the Apprentice”. During the mission to Malachor, The Sith Inquisitors (Ezra, Kanan and Ahsoka) ultimately choose to save the Republic by destroying the Sith Temple, which houses a superweapon.

What happens to Captain Rex?

Is Rex still alive in the Mandalorian?
